Information on accidents, incidents, and accidents involving lifting machines in the Republic of Belarus in 2018

09.01.2018 at the construction site "Residential building 18a in microdistrict 15 of Rechitsa city", during the unloading of rebar cages from the semi-trailer of a tractor unit using a КС-55727-7 automobile crane with a lifting capacity of 25 tons, owned by UP "SvetlSpetsTekhnika" (Svetlogorsk city), the crane overturned. According to the results of the 5th technical investigation into the causes of the specified incident, it was established that the crane overturned as a result of exceeding the rated lifting capacity by more than double at the boom length used during operation, which was made possible due to the intentionally disabled crane load limiter (ENK-160S).

18.07.2018 at 11:00 AM at the construction site "Bridge over the Kamenka River at km 42+415 of the R-115 Vitebsk-Gorodok highway (to the M-8/E95 highway)", during the relocation of compressor equipment weighing approximately 1600 kg using a КС-4561 AM automobile crane with a lifting capacity of 16 tons, owned by the branch "Mostootryad No. 425" of OJSC "Mostostroy", the crane overturned and fell from the bridge. In accordance with the technical investigation report on the causes of the incident, the employees who violated the requirements of industrial safety legislation, which led to the incident, are officials of the organization operating the crane, responsible for carrying out production control and safe crane operations, as well as the mobile crane operator who installed the crane on loose and untamped soil. Considering that the causes of practically every emergency involving lifting cranes are related, among other things, to incorrect actions of slingers and crane operators, during the dissemination of this information on the causes and consequences of past emergencies, as well as during briefings with the specified employees, we consider it advisable to focus on the following restrictions: I SLINGER IS PROHIBITED FROM: - involving persons not qualified as slingers in cargo slinging; - transferring radio communication devices to other persons for transmitting signals to the crane operator; - using faulty, unmarked lifting devices and containers; - slinging cargo whose mass is unknown or exceeds the crane's lifting capacity; - performing cargo binding and hooking in violation of slinging diagrams; - using items and materials not provided for in the slinging diagrams (crowbars, pins, wire, etc.) for binding and hooking cargo; - slinging cargo that is jammed, stuck, covered with soil, frozen to the ground, blocked by other cargo, filled with concrete, etc.; - pulling slings from under the cargo after unhooking (except in cases provided for by technological regulations) using lifting cranes; - releasing lifting devices (slings, grabs, traverses, etc.) jammed by cargo using the crane; - transmittingto the crane operator for lifting and moving a load by crane when people are in the danger zone of the moved load; - to be on the load during its lifting or movement, as well as to allow the lifting or movement of the load if people are on it; - to be under the lifted load; - to be in the body of a motor vehicle during lifting and lowering of loads into the body; - to give a command to the crane operator to lift and lower a load into the body of a motor vehicle when the driver of the vehicle or other persons are in the cabin; - to accompany the load without using appropriate devices (slings, hooks), to balance the load with their own weight; - to be in the zones of moving parts of the crane (counterweight, rotating platform) to avoid being trapped between the rotating and non-rotating parts of the crane. THE CRANE OPERATOR IS PROHIBITED FROM: - operating the crane with faulty safety devices, on faulty crane runways; - intentionally disabling safety devices; - using limit switches as operating mechanisms for switching off mechanisms; - using faulty, unmarked removable lifting devices and containers; - lifting and moving a load that has been slung by an employee who does not have a certificate for the right to service lifting cranes in the profession of "rigger"; - moving or tilting a load whose mass is unknown or exceeds the lifting capacity of the crane, as well as a load that has been slung in violation of slinging diagrams; - lifting a load with people on it, moving a load over people; 6 7 - lifting and lowering loads into the body of a motor vehicle, semi-wagon when people are in the body or in the cabin of the motor vehicle, semi-wagon; - lifting, moving and lowering loads when people are between the moved load and the equipment, when unauthorized persons not involved in crane operations are in the crane's working zone; - operating the crane onat a distance of less than 30 meters from an overhead power line without a work permit; - installing a crane under an active power line without its de-energization; - installing a crane on an unprepared site (freshly filled, uncompacted soil, on the edge of an excavation slope, ditch, with a site slope exceeding that established in the operating documentation). In order to prevent accidents and incidents, and to avoid occupational injuries, we propose to disseminate this information to interested parties.

Information on elevator accidents

In 2018, 2 accidents related to the operation of potentially hazardous facilities (elevators) in industrial safety subjects occurred.

04/02/2018 a fatal accident occurred with an employee of the trade and production complex "Velikolepny Express" (shop No. 29 of OJSC "DORORS") during the loading of cargo into the elevator car. At the time of the incident, the victim was in the gap between the landing stop and the elevator car, as a result of the spontaneous movement of the elevator car, the person was crushed. Causes of the accident: the elevator was operated in a technically faulty condition (violation of paragraphs 75, 152 of the Rules for ensuring industrial safety during the operation of elevators and construction freight elevators, approved by the Resolution of the Ministry of Emergency Situations of the Republic of Belarus dated 03/01/2011 No. 18 (hereinafter - the Rules); the elevator was not registered as a potentially hazardous facility with the territorial department of Gospromnadzor (violation of paragraph 62 of the Rules); the elevator was operated without technical inspection and maintenance (violation of paragraphs 104, 125 of the Rules).
