On the results of the technical investigation of the incident

On July 14, 2024, at the construction site "Apartment building No. 33 on Mira Street in Kalinkovichi," during a storm front, the braces of the tower sections of the KBM-401P tower crane (manufactured in 2009 by

RPUPE "Mogilev Plant "Strommashina"), owned by

OAO "Mozyrpromstroy," were damaged (deformed). Due to adverse weather conditions and the absence of electricity, construction and installation work

using the specified crane at the site during this period

was not carried out.

The investigation established that the deformation of the lifting crane's metal structures occurred as a result of wind impact when the slewing mechanism was not braked. The causes of the incident were the following violations of industrial safety requirements:

1. The instruction for safe work for the tower crane operator (operating instructions) was developed without considering the requirements of the KBM-401P tower crane's operating manual (the requirement to brake the tower crane's slewing mechanism after work cessation is missing);

2. The crane work production project does not provide for safe operation conditions for multiple lifting cranes whose operating zones intersect;

3. On the eve of the incident, after ceasing work, the crane operator did not brake the slewing mechanism of the tower crane.
