Additional information

the following documents are required to receive supported living services:

§  written application for social services;

§  identity document of the citizen;

§  document confirming the right to benefits;

§  consent to the processing of special personal data.

Within three days from the date of receipt of the application, an examination of the applicant's material and living conditions is carried out, based on the results of which an act of material and living conditions is drawn up.

When submitting an application for social services, citizens have the right to independently provide documents requested by the territorial center
