Comprehensive Social Home Care
Social services in the form of home social care (social worker services) are provided to incapacitated citizens — disabled persons of groups I or II, non-working citizens aged 60 and older, who have reached the generally established retirement age, are entitled to a state pension, have self-care impairments corresponding to functional class FC-2, FC-3, FC-4, subject to medical indications and absence of medical contraindications.
Social services are provided based on the List of Free and Accessible Social Services of State Social Service Institutions with Norms and Standards for the Provision of these Services to Citizens, approved by Resolution of the Council of Ministers of the Republic of Belarus No. 1218 dated December 27, 2012.
The list of social services, as well as the norms and standards for the provision of these services to citizens, are determined depending on the degree of limitation of the self-care ability of the applicant, as confirmed by a medical certificate of health.
Social services are provided under the following conditions:
· free of charge – for low-income single incapacitated citizens (families).
· on partial payment terms — for single incapacitated citizens (families) whose average per capita income does not exceed 200% of the subsistence minimum budget per capita (BPM), approved in the established manner.
The amount of partial payment is:
- for an incapacitated citizen: 60% of the tariff for social services;
- for an incapacitated family: 50% of the tariff for social services for each incapacitated family member;
· on full payment terms:
– for single incapacitated citizens (families) whose average per capita income exceeds 200% of the subsistence minimum budget per capita (BPM), approved in the established manner;
- for incapacitated citizens (families), regardless of their income and place of residence of their children;
The amount of full payment is:
- for an incapacitated citizen: 100% of the tariff for social services.
When determining the payment conditions for social services, the following are applied:
– the amount of the subsistence minimum budget per capita (BPM), approved in the established manner, valid on the date of enrollment for social services;
– the average per capita income of an incapacitated citizen (family), calculated for the month preceding the month of application for social services.
The cost of comprehensive social home care for citizens is determined individually, based on the duration of the visit and the amount of the fixed tariff for social services, approved by the decision of the Vitebsk Regional Executive Committee dated September 19, 2024, No. 591 (at the rate of 0.77 Belarusian rubles per hour).

To provide social services in the form of home social care, the applicant submits the following documents:
· Identity document of the citizen;
· A standard document on the right to benefits (certificate of a disabled person, veteran of the Great Patriotic War, etc.) for citizens belonging to the category of beneficiaries;
· A medical certificate of health status and (or) a conclusion of the VKK (Medical Advisory Commission) containing information on the presence of medical indications and (or) the absence of medical contraindications for the provision of social services in the form of home social care (Form 1zdr/u-10); where medical indications for the provision of comprehensive social care services are defined: injuries and (or) diseases and (or) their consequences, leading to moderate (functional class 2 (FC2)), or severe (functional class 3 (FC3)), or sharply severe (functional class 4 (FC4)) limitation of self-care ability;
· Written application;
Department employees, within 3 days from the date of application, conduct an examination of the citizen's material and living conditions with the drawing up of an inspection report and request from state bodies and other organizations:
· Information on the amount of pension received for the month preceding the month of application for social services (for single citizens);
· Information on the absence of care for the citizen who applied for social services by a person receiving a care allowance for a group I disabled person or a person who has reached the age of 80;
· Information on the living quarters occupied in this locality, place of residence and family composition.
Employees of the territorial center, within three working days from the date of application, conduct an examination of the citizen's material and living conditions with the drawing up of an act determining individual need for social services.
The decision to conclude a social services agreement is made by the head of the territorial center within 5 working days after receiving the last document (information) necessary for the provision of social services.
