Caregiver Services
The home social assistance department provides caregiver services from 10 to 40 hours per week to incapacitated citizens — disabled persons of groups I or II, non-working citizens aged 60 and older, who have reached the generally established retirement age, are entitled to a state pension, have lost the ability to self-care and move (having impairments in self-care and independent movement corresponding to functional class FC-4), provided there are medical indications and no medical contraindications.
Social services are provided based on the List of Free and Publicly Available Social Services of State Social Service Institutions with Norms and Standards for the Provision of these Services to Citizens, approved by Resolution of the Council of Ministers of the Republic of Belarus No. 1218 dated December 27, 2012.

Caregiver services are provided:
· on partial payment terms — to low-income single incapacitated citizens whose average per capita income is below the subsistence minimum per capita (BPM), approved in the established manner, valid on the date of application for social services.
The amount of partial payment for an incapacitated citizen is 60% of the social services tariff — 0.72 rubles per hour.
· on full payment terms — to incapacitated citizens, regardless of their income and place of residence of their children, with the exception of low-income single incapacitated citizens.
The amount of full payment for an incapacitated citizen is 100% of the social services tariff.
The tariff for caregiver services is approved by the decision of the Vitebsk Regional Executive Committee No. 591 dated September 19, 2024 (at 1.20 Belarusian rubles per hour).

To receive caregiver services, the applicant must submit the following documents:
· an identity document of the citizen;
· a standard document on the right to benefits (disability certificate, Great Patriotic War veteran certificate, etc.) for citizens belonging to the category of beneficiaries;
· a medical certificate of health status and (or) a conclusion from the medical advisory commission (VKK), containing information on the presence of medical indications and (or) the absence of medical contraindications for the provision of social services in the form of home social care (Form 1zdr/u-10); where medical indications for caregiver services are defined as: injuries and (or) diseases and (or) their consequences leading to sharply expressed (functional class 4 (FC4)) limitations in self-care and independent movement;
· a written application;
Within 3 days from the date of application, the department's employees conduct an assessment of the citizen's living conditions, drawing up an inspection report, and request the following information from state bodies and other organizations:
· information on the amount of pension received for the month preceding the month of application for social services (for single citizens);
· information on the absence of care for the citizen applying for social services by a person receiving a care allowance for a Group I disabled person or a person who has reached 80 years of age;
· information on the residential premises occupied in the given locality, place of residence, and family composition.
Employees of the territorial center, within three working days from the date of application, conduct an assessment of the citizen's living conditions, drawing up an act of determining individual need for social services.
The decision to conclude a social services agreement is made by the head of the territorial center within 5 working days after receiving the last document (information) necessary for the provision of social services.
