Personal Assistant Services

for disabled individuals living with able-bodied relatives legally obliged to support them, who have a severely pronounced limitation in their ability to move independently and/or to orient themselves, corresponding to FC 4, and/or a moderate or pronounced limitation in their ability to control their behavior, corresponding to FC 2 or FC 3 – up to 20 hours per month as needed;

for disabled individuals living separately from able-bodied relatives legally obliged to support them, and single disabled individuals:

who have a limitation in vital activity (ability to move independently and/or ability to orient themselves), corresponding to FC 4 – up to 40 hours per month as needed;

who have a limitation in vital activity (ability to control their behavior), corresponding to FC 2 or FC 3 – up to 60 hours per month as needed

Additional information

the following documents are required to receive personal assistant services:

§  written application for social services;

§  identity document of the citizen;

§  document confirming the right to benefits;

§  consent to the processing of special personal data in cases provided for by personal data legislation;

§  Individual Rehabilitation Program (IRP); 

§  medical certificate of health status, containing information on the presence of medical indications and/or absence of medical contraindications for the provision of social services and/or conclusion of the Medical Advisory Commission (MAC).

Within three days from the date of receipt of the application, an examination of the applicant's material and living conditions is carried out, based on the results of which an act of material and living conditions is drawn up, and the following documents are requested:

§  certificate of residence and family composition, as well as information on the presence (absence) of care for the person applying for social services, by a person receiving a care allowance for a group 1 disabled person or a person who has reached 80 years of age;

Citizens, when submitting an application for social services, have the right to independently provide the documents requested by the territorial center.
