Social Respite

MEMORANDUM

on the provision of the "social respite" service

State Institution "Bogushevsky Boarding Home for Children with Disabilities with Psychophysical Developmental Peculiarities"

1. The "social respite" service is provided for no more than 56 days in a calendar year and no more than 28 consecutive days.

2. The cost of one day of the "social respite" service is - 6 rubles 74 kopecks.

3. When providing this service, pensions and benefits for the child are paid in full.

4. The "social respite" service is provided to children with disabilities with psychophysical developmental peculiarities (neuropsychiatric pathology) aged 4 to 18 years, who need constant external care or external assistance, household services, and medical care.

5. When providing this service, the child in the boarding home will be provided with the following medical procedures: massage, exercise therapy, physiotherapy, thermotherapeutic procedures, relaxing therapy, diet therapy, as well as education, according to the conclusion of the Center for Correctional and Developmental Education and Rehabilitation;

6. Upon admission to the boarding home for the "social respite" service, parents (or one of them, if the family is incomplete) or the legal representative of the child with disabilities must provide the following documents:

-voucher;

-application;

-passport of a citizen of the Republic of Belarus or birth certificate of the child (copy);

- 1 photo;

-disability certificate (copy);

-document confirming the authority of the legal representative;

-parental data (passport data, contact phone numbers, address of residence);

-extract from medical documents;

-medical certificate of health form 1-med/u-10 (for a child being admitted to a boarding home);

-the results of tests submitted at the time of admission to the boarding home must not exceed the established deadlines;

-conclusion of the State Center for Correctional and Developmental Education and Rehabilitation;

-psycho-pedagogical characteristics.
